PhD Survey for a friend

A friend is a registered psychologist, and is writing currently writing her PhD. She is researching relationships between supervisors and employees and also relationships among colleagues in the workplace. She is having some problems getting enough data for her thesis, so if anyone has some spare time, would you consider filling out the survey for her? It will take about 10-15 minutes to complete.

Thats an old one though. I remember completing that for a company I used to work for.
Turns out the "Supervisor" I used to work for was the biggest prick ever - even though we all really respected him and tried to dot he best for him!
He got the promotion! :D
